Provider Feedback on New and Updated Forms

The Department of Health has developed a range of new or updated documents, forms and factsheets following the introduction of the new Hearing Services Program (Voucher) Instrument 2019 and Service Provider Contract, and in response to the feedback received in the Provider Support Survey conducted in July 2019.

Further documents will be published between now and 31 December 2019.

Why your views matter

To ensure these forms will support providers to comply with program requirements, we are asking for your feedback on their use.  Feedback can be provided anonymously or you can give us your contact details if you would like us to respond to you.

What happens next

We are reviewing all feedback provided and will update forms as required.  All providers will be notified of changes to the forms via the website and updated forms will be provided to software manufacturers.
