Introduction

Who should complete this NDIS PPE request application?

For efficiency, this application is best completed by someone who can provide details about the provider or the participant, the type of services received or delivered, attempts to source commercial supply of PPE and the essential PPE needs for care delivery. 

If you are an NDIS provider, please complete an application for each service location/site. If PPE is supplied, it will be distributed directly to where services are delivered (NOT a head office location). Delivery location and instructions are collected in the application.

Application Assessment and Response

After you have submitted the application you will receive contact from a Department of Health and Aged Care officer. The officer will:

  • request evidence of your attempts to source PPE from commercial suppliers
  • seek further information, such as the frequency and type of care being delivered, to understand your request, and
  • advise the outcome of the application. 

Notice of Collection

Any personal information you provide to the Department of Health and Aged Care, such as your name and contact details, will only be used for the purpose of dealing with your request for PPE from the National Medical Stockpile.

Information being collected about you will be used and disclosed in accordance with the Privacy Act 1988. Where applicable, your name and contact details will be disclosed to the National Disability Insurance Agency and delivery providers managing PPE delivery or Australian Government agencies with National Disability Insurance Scheme policy and regulatory responsibilities.

The Department of Health and Aged Care’s privacy policy outlines how you can check that our records of your personal information are correct and how you can complain if you think that we have not dealt appropriately with your personal information.

RAT Kits

From 1 February 2023, the NMS no longer supplies RATs to residential disability support providers.

NDIS participants can continue to use their core funding (at no cost to the participant) to purchase RAT kits where they are needed to safely access their reasonable and necessary supports for themselves and for workers that are delivering supports for them.

Eligible NDIS providers can also claim for RATs when a support worker has to undergo COVID-19 testing to safely deliver supports to a participant, as part of ongoing COVID-19 support measures.
