Who should complete this Aged Care PPE request application?

For efficiency, this application is best completed by a senior person in the organisation who can provide details about the organisation, the type of services delivered, and the PPE stocks and PPE needs for care delivery. It will step you through key considerations about your request.

Please complete an application for each service location/site. PPE will be distributed directly to where services are delivered (not a head office location). Delivery location and instructions are collected in this application.

If you are NOT in a priority category, it is unlikely that we can help you at this time. You may decide not to complete this application.

Notice of Collection

Any personal information you provide to the Department of Health, such as your name and contact details, will only be used for the purpose of dealing with your request for PPE from the National Medical Stockpile.

Information being collected about you will be used and disclosed in accordance with the Aged Care Act 1997 and Principles and the Privacy Act 1988. Where applicable, your name and contact details will be disclosed to a relevant State and Territory Government and delivery providers managing PPE delivery.

The Department of Health’s privacy policy outlines how you can check that our records of your personal information are correct and how you can complain if you think that we have not dealt appropriately with your personal information.
