Bendigo Health to Residential Aged Care Homes (RACH) discharge checklist – Hospital staff evaluation survey

Overview

Bendigo Health has implemented a new Residential Aged Care Discharge Checklist, which is intended to be completed for every patient transitioning from hospital to a residential aged care home. The checklist aims to support safe, timely, and consistent discharge planning by ensuring key clinical and care information is communicated effectively at the point of transfer.

We are seeking feedback from staff involved in completing or using the checklist to evaluate its usefulness, practicality, and effectiveness in supporting patient transitions to residential aged care. This survey will take less than 5 minutes to complete, and all responses are anonymous. Your feedback will help inform future improvements to the checklist and the discharge transition process.
