Thank you for your interest in becoming accredited as a service provider for the Australian Government Hearing Services Program. 

You may wish to download a PDF of the application form for your information before you proceed. Note that we will not accept applications that have been submitted as the PDF form. All applications must be submitted through this online application form. 

Most applications are processed within 20 business days.
Processing will take longer where required information or documentation has not been provided.
