Provider Details

Please complete the provider details below and certify they are correctly listed in the Hearing Services Portal (the portal), under Service Provider Details, Contact Details and Address Details.

Please be aware that you will require SP Admin access to view these details.

Contracted Service Provider (provider) Name

This is the name of the business entity that has signed a contract with the Department of Health, Disability and Ageing.

Enter the provider name as shown under 'Organisation Name' in the portal.

Contractor Code (provider number)

This is the number listed under 'Contractor Code' in the portal.

The Contractor Code is a four-digit number and starts with a zero (0).

Please do not enter a Qualified Practitioner or Site ID number.

If you are unsure of the correct Contractor Code, please contact us on 1800 500 726 or Hearing@Health.gov.au.

Are the Contract Holder details for the provider correct in the portal?

Please confirm the details of the main contact person and the main contact email for the provider. This should be for the person listed as 'Contract Holder' in the portal.

The Contract Holder is usually the primary contact for the provider and does not necessarily need to be the person completing the SAT.

Are the provider address details correctly listed in the portal?

The contact address details should be the address listed in the portal under the Address accordion.

This will usually be a Head Office or main permanent site address.

Provider Australian Business Number (ABN)

Enter the ABN for the provider.

If the provider is a Trustee, this will be the ABN of the Trust, not the Trustee.

If you are not the Contract Holder, please check with the Contract Holder.

Is the provider a Trustee for a Trust?

If you are not the Contract Holder, please check this with the Contract Holder.

If unsure, you may need to confirm with your accountant.
