Overview

We want to hear from First Nations organisations and stakeholders of the Integrated Team Care (ITC) program about the proposed approach and timeline for transition to the First Nations community controlled sector.

The ITC program is a flagship national program funded under the Indigenous Australians’ Health Program (IAHP) since 2016. The ITC program aims to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care.

The Department of Health, Disability and Ageing (the department) is working with the First Nations health sector to determine a path to transition funding arrangements for the ITC program to the Aboriginal and Torres Strait Islander community controlled sector.

We think the survey will take about 15 minutes to complete. We encourage participation from organisations, groups or alliances with relevant experience or a vested interest in taking leadership of ITC in future models.

All data will be securely stored and used solely to inform improvements to an expression of interest process. Individual responses will not be shared externally. Your personal information is protected by law, including the Privacy Act 1988 (Privacy Act) and the Australian Privacy Principles (APPs), and is being collected by the department, via Citizen Space, for the purposes of informing the development of an expression of interest process for the ITC transition and grant process. Your answers to the survey may include sensitive information such as, your organisation name and your perspective in completing this survey (i.e. is your organisation interested in becoming an “Early Adopter” and responding to the future EOI). The department will collect your personal information at the time that you complete the survey, however, you can terminate the survey at any time prior to submitting. The department’s Privacy policy outlines how we comply with the Privacy Act 1988 and what to do if you think we have mishandled your personal information.

Why your views matter

By sharing your views on a proposed expression of interest (EOI) process for identifying organisations interested in leading the commissioning of ITC, the department can improve the expression of interest process, including its timing, support materials, and any other considerations you would like to contribute.
