Residential Disability Accommodation COVID-19 Winter Dose Vaccination Clinic - Registration Form

Closed 31 Dec 2022

Opened 28 Mar 2022

Overview

Winter doses for residential disability accommodation settings can be sourced through a variety of channels, including primary care providers such as GPs and pharmacies, working with Primary Health Networks (PHNs) to identify primary health care in-reach options and Commonwealth in-reach. Where a disability service provider is not able to source a primary care provider to administer COVID-19 Winter Dose Vaccine, they are able to request an in-reach Commonwealth COVID-19 Winter Dose Vaccination Clinic.

To be eligible for a Commonwealth COVID-19 Winter Dose Vaccination Clinic:

  • A disability service provider will need a resident or residents requiring a COVID-19 vaccination.
  • Resident/s requiring a vaccination can include those requiring a winter dose as well as a dose 1, dose 2 or first booster dose of the COVID-19 vaccine.

Note that dose 1, dose 2 and the first booster can also be administered for staff, if this can be accommodated in the disability residential accommodation setting.

Disability Service Providers are responsible for requesting the COVID-19 Winter Dose Vaccination Clinic on behalf of their residential disability accommodation setting/s and should complete the form to opt-in and register interest for a Commonwealth in-reach COVID-19 Winter dose clinic. Please note that disability service providers do not need to complete one form for each setting and can submit a request for multiple settings on one form.

Please note that the Commonwealth clinics will only deliver COVID-19 vaccines. Co-administration of the Winter dose and the 2022 flu vaccine is however, recommended by ATAGI. You will be required to make separate arrangements for administration of flu vaccines unless:

  • you have a contracted commercial provider to administer your annual flu vaccinations, and this provider is also contracted by the Commonwealth to deliver in-reach COVID-19 Winter Dose Vaccination Clinics in your area.
  • this provider agrees at your direct request to administer both vaccine programs at the same time. You will be required to organise the flu vaccination component directly with that provider.

If you have any questions about the registration form or the opt-in process, you can send your questions to DisabilityCovidVaccineDelivery@Health.gov.au

As with the first booster program, Disability Service Providers are strongly encouraged to engage with the Winter dose program and take the necessary steps to prepare and ensure that they have identified arrangements for COVID-19 Winter Dose Vaccinations for when residents become eligible.
