There is no current risk management component in accreditation standards. Need to consider impact on falls/ injury and potential coroners cases and breaches of duty of care when care recipients place themselves at risk . What are the processes related to risk management and harm minimisation. What will be the impact on duty of care for an organization?

Different quality systems across residential and home care create different clinical and management foci. High clinical overlay in residential care out balances the clinical needs over lifestyle and care recipient driven service delivery and needs to be addressed - however the care recipients that are in the services are more frail with higher acuity of need. . • Would like to see: o Literature review of best practice referenced in the Rationale and Evidence section of each standard o Clarification around Consumer goal and preference outcomes - how to objectively assess a Consumers perception against a standard - where perception may not match with reality o Diagram of standards clarified - assessment and planning in an arrow surrounding the Consumer and care delivery standards gives the appearance that the standard is less relevant to the consumer in the center of the diagram o Removal of standard 6 Feedback and Complaints – this could be rolled into another standard e.g. standard 8. To dedicate one standard to a single system, when other standards like 3 care/ clinical and 8 governance include multiple systems, results in uneven weighting of systems across standards. Particularly when the Aged Care Complaints Commissioner has a regulatory role in complaints already. The aim of the new standards is to reduce duplication not add to it. o Optional standards need clarification – how will the standard be applied across an organisation with multiple services delivered e.g. standard 5 “not intended to apply to home care” but will apply to RAC Further information needed around how the options apply to organisations with multiple delivery sites and multiple aged care programs, with different risk profiles. For example, would the Governance standard be applied once only across the organisation or at each site? Will this be managed with a central single audit or duplicated at each site? prefer that the governance and HR standards are managed with central/ corporate office with demonstration at the sites.

Perception of care recipients may impact on reality and experience of care recipient - particularly with diagnosed dementia or other memory issue - therefore they may be excluded from monitoring and their experiences are not considered - unless by a family/ carer with or without own experiential biases No philosopher using deduction or researcher using brain scans has been able to prove with certainty that our perception of the world matches "reality as it is out there."
