15. What features of the existing assessment and monitoring process should be retained?

Retain assessment & monitoring processes
I dont think accrediation actually captures of what is happening at a facility... workers are too scared to speak up of issues.. extra staff are often put on to ensure on the day things run smoothly. Additionally often things that are bought up as issue for the accrediation people are change for a short time to demonstrate willingness to act but go back to the way there were before once accrediation is finished.

16. What features of the existing assessment and monitoring process should be changed?

Change assessment & monitoring
More monitoring.. Nurse patient ratios seriously this is needed. Acfi update to actually represent to complex needs of the resident More focus on the holistic needs of residents rather than just meeting the bare essentials.

Additional comments

27. Do you have anything else that you would like to contribute to the Review?

Additional comments
Yes you are attmepting to look at this issue from an upstream approach once the issues arrive... yes we need meaure to ensure aged care facilities are meetinf standards set out and there are complaint systems in place but a lets face some facts we need to address issue prior to they come this can only be done by a paradigm shift in aged care included a more holistic approach as well as correct ratio of staff to patients.
